To be cleared, you need to have at least a year
clean with no felony convictions for at least three years. You
cannot be on probation, or have been institutionalized in the past
two years. This is checked with fingerprints eventually so be
honest. If you cannot meet these qualifications, I am sorry, but a
power much greater than me cannot waive them.
If you think you can meet the previous
qualifications, then you need to print out each page of the
application by clicking on this link and printing each page.
Push the print button at the bottom of the page. After
printing, fill out the application and mail it to JUST @ the
address on the last page of the application.
Expect to wait a couple months for a response.
